1. Fill out a Pre-Registration Questionnaire.
2. Receive your Access ID(s) and Access Password(s) for each child via email.
3. Create a Parent Portal Account (instructions in your email or click here to access instructions).
4. Navigate to “Forms” and select “Enrollment” and fill out all enrollment forms.

Student’s Birth Certificate
State of Illinois Certificate of Child Health Examination and/or Immunization Records (required for all New Students, and all PreK, Kindergarten, and 6th grade students)
Illinois State Board of Education Student Transfer Form (from previous school) and/or Last Report Card (if transferring from out-of-state)
Go to the Parent Portal and select “forgot username/password.” The reset instructions will be sent to the email that we have on file. If you no longer have access to this email account, please contact the main office.
Please complete filling out all the required registration forms. Contact the main office and someone can assist you with your document attachments.
We will be offering on-site open registration days throughout the Spring and Summer. Please plan to attend or contact the main office to make an appointment for an alternative time.
